                      Stressful Conditions

There are frequently day-to-day conditions that we find stressful. 
Go through the list below, reading each one, and put a check next
to those that apply to you.  Then go back over the checked items
and indicate how often each source is true for you by circling the
appropriate number (1 = very true...5 = untrue).
